If you want to become an agent, there are a few simple steps to follow:

1. Fill out an Agent Application Form and Questionnaire (below)

2. Heartland preliminary approves new agent

3. Agent fills out and signs Heartlands Agent contract

4. Agent becomes authorized to represent Heartland

5. Heartland will send Marketing Materials and provide support as required
